National Consultant (Consultancy for Supporting National Oxygen Capacity Building in Bangladesh)

A. Objectives:

The objective of this consultancy is to contribute to supporting the national oxygen capacity building and oxygen security in country through the finalization of the National Medical Oxygen Plan for the Ministry of Health and Family Welfare (MOHFW) and its implementation and monitoring.

B. Background:

Bangladesh is currently facing an increase in the number of COVID 19 cases over the last few weeks. Being seen as the second wave of COVID 19, there is increasing concern on this rise in cases as well as the country’s capacity towards effectively responding to the pandemic. Urgent catalytic support had been made to improve the national capacity for provision of medical oxygen to meet the increasing needs amid the surge of COVID-19 cases in Bangladesh and also enhancing the capacity of health facilities  in oxygen management system within the facilities.

The insurgence in COVID19 cases following the second wave in Bangladesh has put tremendous pressure on the already stressed health system for COVID19 case management, particularly managing the oxygen requirement which has increased significantly (almost four folds) over the last four months. There are gaps in defining the oxygen landscape in Bangladesh and bringing together all the available data on oxygen production and consumption for the medical sector including an understanding of the industrial sector. The oxygen landscaping exercise will facilitate the finalization of a national medical oxygen plan for the country. This will facilitate and guide policy makers, programme managers and the MOH&FW and partners and all stakeholders towards decision making and actions for building oxygen security including the rational use of oxygen not only during the COVID 19 response but for the long term taking a systems approach. 

UN Agency  is providing support to the MOHFW for facility readiness including oxygen capacity building for safe and rational use of oxygen within the quality improvement initiative. Work is also ongoing towards improving maternal, newborn and child health outcomes. However, there is need for further contextualized capacity building at national and sub national levels including the facility level. This needs to be linked to the operationalization of the national medical oxygen plan and the guidelines towards the rational use of oxygen in the country.  There is need to work closely with the MIS team in the Directorate General of Health Services (DGHS) for aligning the available data and identifying the gaps in data for supporting evidence-based planning and informed decision making on oxygen system and capacity development in Bangladesh in a sustainable way.

C. Scope of Work:

Under this contract, the consultant will be responsible for completion of the following deliverables. He will collaborate with the MOHFW and DGHS and other key stakeholders and partners at national and sub national levels to provide technical assistance towards completing the development of Oxygen Landscaping in Bangladesh and also development of the “National Medical Oxygen Plan” for the Ministry of Health & Family Welfare including supporting the operationalization of the plan as needed through the a National Policy Framework for improved national system and capacity for provision of adequate and safe Medical Oxygen for health facilities in Bangladesh.

At the same time consultant will also work with the MIS team towards facilitating the collation of data, including capacitating  the national HMIS unit of the DGHS for improved data system and data analytics on Oxygen needs and uses for real-time tracking and evidence-based decision making.

The final deliverable will be –

  1. Development of Oxygen Landscaping in Bangladesh: 45 working days
  2. National Medical Oxygen Plan including improved data system and data analytics on Oxygen needs and uses for real-time tracking and evidence-based decision making: 30 working days

D. Tasks:

The Consultant will be engaged to support one or several of the following priority activities:

  • Bringing together all the key data elements on oxygen in the country to understand ad finalize the Oxygen Landscape report.
  • Complete a needs assessment towards understanding the national and sub national capacities towards the rational use of oxygen with the MOHFW and key stakeholders.
  • Provide technical assistance to develop the “National Medical Oxygen Plan” of Ministry of Health and Family Welfare (MOH&FW) based on the national oxygen landscape.
  • Provide technical assistance to the MOHFW in-terms of policy framework to operationalize the “National Medical Oxygen Plan”.
  • Provide technical assistance to the MOHFW and DGHS to update the National Oxygen Guidelines, tools and modules for operationalizing the National Oxygen plan at service delivery platforms along with programme capacity building.
  • Facilitate the development of operational guidelines for the programme managers and service providers for both building oxygen capacity and safe and rational use of medical oxygen for hypoxaemia management.
  • Quality assurance process established for operations and maintenance of the oxygen supply system in selected hospitals as per national guidelines.
  • Work with the current quality improvement interventions and synergize the tracking, monitoring, evaluating, and improving systems for oxygen provision and use case management at all levels of the healthcare system, through mentorship and supportive supervision.
  • Support the HMIS team to integrate Oxygen indicators in the national HMIS (DHIS2) and dashboard and capacity building of the national HMIS team on data analytics for oxygen for decision making.
  • Support exploring innovative ways of capacity building and mentoring on oxygen use including the digital platform and setting up regional hubs as well as mobile teams as needed.
  • Support building a partnership framework for oxygen security in the country bringing together the public and private sector and all key stakeholders at national and sub national levels.
  • Facilitate the finalization of a robust supply chain systems for oxygen including transportation, refilling and maintenance ensuring these are operationalized in country.
  • Provide support to the oversight and tracking of the ongoing liquid medical oxygen work in 30 hospitals in the country and ensuring these hospitals have robust oxygen security systems in place.
  • Provide support to all procurement ongoing and in the future for oxygen related equipment.
  • Performs other tasks as assigned.

F. Consultancy Costs:

  • The consultant will get BDT 45,000 daily basis as a consolidated package which includes lumpsum travel cost.
  • Computer and communication will be borne by the Consultant
  • Stationary and other office equipment will be borne by the Consultant
  • Consultant will be responsible to pay all taxes as per government rules and regulations. E-Zone will make statutory deductions towards income tax as per government rules and regulations.

G. Profile Requirements & Qualification Skills

  • Qualifications can include a medical degree, masters in public health, masters in social sciences, bio medical engineering and other relevant areas
  • At least 8 years working experience in supporting programs that link with Oxygen systems, national policy, strategy and guidelines development on related health areas
  • Preference will be for expertise and knowledge on conducting national oxygen capacity and need assessment, prior experience in working with oxygen related programs, developing national Oxygen plan for the Ministry of Health in a Lower Middle Income country
  • Familiarity with Oxygen infrastructure and capacity building processes including standards for medical Oxygen gas pipelines system, i.e. LMO/VSA/PSA system including regulatory and safety compliance issues is an added asset
  • Well versed in computer programmes like MS office
