Senior National Consultants (WASH in HCF Implementation Plan Development – Lead and Cost Consultant)

Duty Station: Dhaka

Contract Duration: Lead (3 Months) and Co-Lead (1 Month)

Reporting to: Health Specialist, Dhaka

A. Objective

The objective of hiring two national consultants is to provide technical assistance to MoH&FW/DGHS/DGFP for development of Costed Implementation Plan of the recently approved “National Strategy for WASH in Healthcare Facilities and A Framework for Action”.

This consultancy will contribute for operationalization WASH in HCF Strategy and Framework of Action and ultimately achieve the results for improving the quality of care, decreasing health facility-based sepsis, antimicrobial resistance and improve respect and dignity of clients particularly the mothers and their young babies.

B. Background

Bangladesh had progressed well on reduction of maternal and under five deaths over the last two decades. The current indicators show slow progress on reduction of these deaths (MMR– 176/100000 live births, Neonatal Mortality Rate 23/1000 and Under five 38/1000 live births).  These deaths are related to poor quality of care as 50% of maternal and newborn deaths occur during the process and first 24 hours of delivery. Bangladesh needs to accelerate the reduction on deaths of mothers and children to achieve he Sustainable Development Goal (SDG) 3.1 and 3.2 targets.  Poor or non-functional WASH services in health facilities contributes for compromising not only the quality of care but also the dignity of the patient who seek services from the health care facility.

While the achievement of the SDG 6.1 and 6.2 for safe drinking water and inclusive and equitable sanitation is the realization of the human right, it also contribute significantly to the achievement of the health related SDG 3.1 and 3.2 Water, sanitation, hygiene (WASH) and environmental conditions in healthcare facilities (HCFs) have been neglected areas of concern despite their associated risk with infection, antimicrobial resistance (AMR), morbidity and mortality.  Limited access to improved WASH in HCFs has a direct impact on quality of care (QoC) and maternal and child survival.   A review of the few published studies showing the effects of WASH on maternal and child health emphasized that inadequate sanitation in HCFs was a major cause of dissatisfaction among patients, and a deterrent to women seeking institutional delivery and care.   The growing body of literature on WASH in HCFs and maternal and newborn health points towards the need for an enabling policy environment, nationally-mandated indicators, internal regulation, and independent quality control for WASH in HCFs as essential steps in improving healthcare delivery services.  In this context, the Government of Bangladesh with technical assistance of UN agency and other partners has developed “National Strategy for WASH in Healthcare Facilities and Framework of Action for 2019-2023”. The WASH in Health Care Facility Strategy and Framework for Action is a critical step towards ensuring that all health care facilities have adequate water, sanitation and hygiene services as a contribution towards optimum quality of maternal, newborn and child health services, infection prevention and control, and reduction of anti-microbial resistance and safe health care practices. This, in turn, will enable Bangladesh to achieve its ambition of quality universal health, water and sanitation coverage by 2030.

In this context, the government has requested to the UN agency to provide technical assistance with support of two National Consultants (Senior WASH in HCF Consultant and Cost Consultant) for the development of detail costed implementation plan of the strategy for the next five years.  These two national consultants will work with MoH&FW, MoLGRD&C, DPHE and other partners to develop the detail costed implementation plan for “National Strategy for WASH in Healthcare Facilities and Framework of Action for 2019-2023”.

C. Scope of work

The two consultants will perform the specific tasks for development of costed implementation plan in collaboration with MOH&FW/DGHS/DGFP, MoLGRD&C/DPHE, UN agency, WHO and other partners.

  • WASH in HCF Consultant (Lead): UN agency will engage a Senior National Consultant at NOD level as a lead consultant to provide technical assistance to MoH&FW for development of detail implementation of approved “National Strategy for WASH in Healthcare Facilities and Framework of Action for 2019-2023”.
  • Cost Consultant:  UN agency will also engage a National Consultant preferably Health Economist at NOC level to work with lead consultant for costing of the detail implementation plan.

Accomplishments of these tasks require knowledge and skills of WASH system specifically related to health facilities of Bangladesh. The lead consultant needs to be well acquainted with the Public Health Engineering system with focus on WASH System to health care services. The consultant should be expert on development of implementation plan of public health intervention with focus on WASH and health system waste management. The cost consultant needs to be health economist with specific skills of quantity surveyors of public health interventions.  Both consultants will work with MoH&FW/DGHS/DGFP and MoLGRD&C/DPHE and other partners for development of costed detail implementation plan of WASH in HCF strategy. Hence these activities and deliverables do not come under the job of the existing staff member of UN agency. The work on development of WASH in HCF costed implementation plan needs specific qualification, skills and expertise. That is why it is proposed to hire two senior national consultants (WASH in HCF consultant as lead and Cost Consultant as Co-lead) with experience of working on WASH sector and public health economics for the cost consultancy.

    2. Two or three Consultative workshops with Ministries (MoH&FW/DGHS/DGFP, MoLGRD&C/DPHE) and other stakeholders, to get feedback on draft implementation plan.

F. Reporting and Deliverables Requirements
The consultants to report to the Health Specialist (HSS) Specialist, Dhaka and will provide following deliverables:

  1. Framework of detail implementation plan along with activities at national and all levels of health facilities (Primary, Secondary and Tertiary);
  2. Approved detail implementation plan of WASH in HCF;
  3. Draft costed implementation plan with incorporation of feedback;
  4. Final costed Detail Implementation Plan of WASH in HCF Strategy

G. Take-home pay

WASH in HCF (Lead) Consultant: BDT 300,000 per month consolidated; BDT 900,000 for 3 months

Cost Consultant: BDT 250,000 per month consolidated; BDT 250,000 for 1 month

H. Profile Requirements, Qualifications, Skills

WASH in HCF (Lead) Consultant:

Education:

  • Postgraduate University Degree Engineering and Environment Health, with focus on WASH

Work experience:

  • At least 8 years of senior management experience in WASH and Environmental Health related government policy, planning and administration;
  • Experience of working on development of policy, strategy and detail implementation plan with focus on WASH and Public Health;
  • Experience on health facility-based assessment focusing on quality of care, infection prevention and control and medical waste management;

Language requirements:

  • Excellent spoken and written skills in English and Bangla.
  • Effective communication and analytical and writing skills.

Cost Consultant:

Education:

  • Postgraduate University Degree on Health Economics   and Public Health, with focus on quantity surveyors of public health interventions

Work experience:

  • At least 5 years of experience in cost consultancy of health interventions strategies and action plan
  • Experience of working on development of health strategies and detail implementation plans;
  • Experience of working with government health system with focus of costing of public health/WASH interventions.  

Language requirements:

  • Excellent spoken and written skills in English and Bangla
  • Effective communication and analytical and writing skills.

I. Administrative Issues

  • Consultant will be located at Dhaka and report to Health Specialist (HSS), BCO- Dhaka
  • WASH in HCF (Lead) Consultant will get BDT 300,000 per month for three months and Cost Consultant will   BDT 250,000 per month for one month as consultancy fee; the travel and DSA will be actual as per UN agency’s consultancy regulation;
  • Computer and communication will be borne by the Consultant
  • Consultants should find own working space;
  • Stationary and other office equipment, will be borne by the Consultant
  • Travel cost, travel arrangement and accommodation need to be arranged in the field by consultant and will be reimbursed as actual cost.
