National Consultant: SBC Strategy development for Urban Primary Health Care -NOC level

Duty Station: Dhaka, Bangladesh. Field visits in 4 city corporations (DNCC, DSCC, Gazipur City Corporation and Narayanganj City Corporation)

Contract Duration: 4 Months from the signing of the contract.

Reporting to: Social and Behaviour Change (SBC) Manager, UN Agency, Bangladesh.

A. Purpose

The purpose of the assignment is to hire a National SBC Consultant, who will provide technical support for developing SBC strategy for Urban Primary Health Care (PHC) in consulting with Health and Nutrition section of UNICEF, relevant counterparts, implementing partners, beneficiaries, and other stakeholders. Specifically, the consultant will do the following:

  • Undertake a comprehensive desk review related published and unpublished data, national and county strategies, guidelines, policies, best practices, materials, and messages to identify gaps and recommendations
  • Facilitate a consultative process to get inputs from key stakeholders and urban PHC beneficiaries on problems, barriers, desired behaviours, and strategic approaches of SBC issues
  • Develop and finalise SBC strategy on Urban Primary Health Care incorporating feedback from stakeholders

The hiring the consultant will follow a UNICEF standard competitive process.

B. Background:

Bangladesh is one of the countries in the world with the most rapid urbanization and fastest growth of slum areas. Currently around 37 percent (62.5 million) of Bangladeshis live in urban areas and in the next 30 years (by 2050) this will almost double, reaching 117.8 million. Social services such as health, nutrition and education are not keeping pace with urbanization, and people - especially the poorest, migrant slum dwellers - are left behind in accessing their basic rights to services, including Primary Health Care (PHC).

The urban health service delivery system is fragmented in Bangladesh; and the poor and socially segregated groups like transport workers, garment workers, rickshaw-pullers & people with disabilities cannot afford to seek health care, not only due to financial hardship but also due to the mindset of service providers. Also high-level of inequity and various behavioural and economical determinants are the other barriers of using health care in urban. There are gaps in clear strategies and coordination amongst the line ministries (Ministry of Health and Family Welfare (MOHFW), and Ministry of Local Government, Rural Development and Cooperatives (MOLGRD&C)) and Local Government Institutions (LGI) for transition from continuing project mode to a programme model of urban health service delivery. Piecemeal, fragmented and donor dependent PHC services for urban Bangladeshis has compelled the population to seek health services from the mushrooming private sector, thereby incurring high out of pocket expenses at around 70 percent.

For improving the Primary Health Care (PHC) services and address the unmet health needs of urban population mostly socio-economically vulnerable including adolescents and women, UNICEF has been piloting six model clinics at four city corporations (Dhaka North City Corporation, Dhaka South City Corporation, Narayangonj City Corporation and Gazipur City Corporation) partnering with government (LGIs), NGOs and private sector service providers.  UNICEF SBC team is also the part of technical team to support implementation promoting the right to health, especially for women and vulnerable groups, help to improve key lifesaving knowledge and skills, increase the demand for a healthy life, as a part of preventive and promotive health and nutrition care.

Social and Behaviour Change (SBC) is a strategic way of communication that aims to empower both individuals and communities to make desired changes that integrated with evidence, capacity building, community engagement and multimedia approach. UNICEF, as an agency with commitment to achieving transformation of communities by addressing Social and Behaviour Change (SBC) issues to  improve health, nutrition and other key social outcomes for children and their families. In Bangladesh Country Programme (CP) 2022-2026, SBC is positioned as a key cross-cutting element and a change strategy for realizing several country programme results. We do this through innovative and evidence-based interventions that address change at multiple levels and contribute towards SBC policy development and practices addressing the local context. 

This TOR is aimed to engage a consultant to develop an SBC strategy which will be informed by the results from the formative and Human Centred Design (HCD) research. The strategy will provide solutions on addressing the deep insights and motivations (attitudes, beliefs), norms (perceived social cultural, gender, religious norms), ability to act (skills, self-efficacy, access), information (knowledge) and barriers hindering the uptake of targeted services among targeted beneficiaries generated from the study. The strategy will also provide solutions on addressing the complex interplay between individuals (self), relationship (interpersonal-partners, family, peers), community (services, products, organizations) and enabling environment (policies, economy, religion, technology).

C. Objectives, Purpose, and Expected Results

Given the background and the SBC Rolling Work Plan 2023-2024, Social and Behaviour Change (SBC) section of UN AGENCY Bangladesh is looking for an expert profession as consultant at NOC level to provide technical support for developing an SBC strategy for Urban Primary Health Care (PHC).  S/he will work under the direct guidance of the SBC section and in close consultation with UN AGENCY Health and Nutrition section, relevant counterparts, stakeholders, beneficiaries, and implementing partners of Urban PHC Model Clinic, to:

  • Undertake a comprehensive desk review related published and unpublished data, national and county strategies, guidelines, policies, best practices, materials, and messages to identify gaps and recommendations
  • Facilitate a consultative process to get inputs from key stakeholders and urban PHC beneficiaries on problems, barriers, desired behaviours and strategic approaches of SBC issues
  • Develop and finalise SBC strategy on Urban Primary Health Care incorporating feedback from stakeholders

D. Scope of Work:

  • Organise need based joint consultation with relevant UN AGENCY section focal and government counterparts and non-governmental agencies at the inception phase and whenever required.
  • Undertake a comprehensive desk review of Primary Health Care and Essential Service Package related published and unpublished data, strategies, guidelines, policies, best practices, materials, and messages to identify gaps and recommendations.
  • Conduct mapping of relevant published and unpublished data, national and county strategies, guidelines, policies, best practices, materials, and messages related to urban primary health care, Essential service package, SBC and community engagement contents including multimedia communication packages and campaign contents on key behavioural issues.
  • Facilitate a consultative method (FGD, KII, In-depth Interview etc.)  to get inputs from key stakeholders, urban PHC service providers and beneficiaries on key problems, desired behaviours, barriers, facilitators, key audience segmentation, message themes and content, and strategic approaches among other issues.
  • Based on the desk review and consultations findings and behavioural insights design a draft SBC strategy
  • Develop message themes and content for each technical area identified with key messages, benefits and call to action.
  • Involved the core team of UN AGENCY including Health and Nutrition sections to harmonise and collate feedback to prepare final draft of the strategy
  • Design and facilitate a workshop with Government counterparts, key stakeholders and implementers/ providers of urban health and nutrition to get inputs sharing the strategy
  • Revise the draft SBC strategy after obtaining feedback from the stakeholders
  • Finalise the SBC Strategy based on feedback from the second stakeholders’ workshop

F. Profile Requirements Qualification Skills:

Consultant must have following criteria to do the assignment

  • Educational qualifications: Postgraduate in Socio-behavioural sciences/ Public Health/ Journalism/Communication for Development and related fields
  • Have extensive understanding about Social and Behaviour Change, Community Engagement and participatory design and planning
  • At least 8-10 years progressive working experience in communication for development field and implementation of social and behaviour communication activities
  • Experience in conducting qualitative research on health, nutrition, and Knowledge Attitude Behaviour and Practice
  • Work experience in community engagement activities (working with urban committees, groups, volunteer, club members, adolescents etc.)
  • Have knowledge on Government Health Service delivery system, and management including coordination amongst the line ministries MOHFW), MOLGRD&C, LGI etc.,
  • Work experience in developing and guiding campaign material development
  • Track record of working with UN, Government.

G. Consultancy Cost:

  • The consultant will get BDT 300,000.00 per month consolidated (inclusive of lumpsum travel allowance).
  • Consultant will be responsible to pay all taxes as per government rules and regulations. E-Zone will make statutory deductions towards income tax as per government rules and regulations  

H. Administrative Issues:

  • The consultants will report to the SBC Manager, UN AGENCY and submit the deliverables outlined under-reporting requirement.
  • For payment by E-Zone on due time, the Supervisor will forward the certified attendance sheet and confirmation of receiving expected deliverables for the respective period.
  • If the consultants required to travel outside their duty station, prior approval will be required, then travel costs will be reimbursed at actuals and DSA will be provided at 2,500-taka rate per day.
  • Consultant will be responsible to pay all taxes as per government rules and regulations.
